
COVID-19 Economic Relief Restaurant & Hotel Fund Application
This fund provides a one-time $1000 payment to eligible restaurant and hotel workers who have been negatively impacted due to the economic effects of the COVID-19 pandemic.
Applicants must provide:
a) Personal Information: Full legal name, DOB, mailing address, social security number, phone number, and email address
b) Business Information: Employer name, manager name, business contact information, date of hire, and date of separation (if applicable)
c) Proof of Employment: Paystub showing proof of employment between October 1, 2020-December 31, 2020 and letter from employer, on company letterhead, verifying date of separation or reduction in number of hours between October 1, 2020-December 31, 2020
